Probationary Members Form
This form should be completed by your chapter FEO immediately after Induction of your Probationary Members. Hitting “submit” once completed will transmit it directly to the VP Collegiates.

Remember to have each member complete the Collegiate Member Profile upon their initiation and send these into the National VP Collegiates. All Initiation Fees should be sent to the National Treasurer.
