Chapter Dues Form
The chapter dues form and ARD form should be completed by your chapter treasurer and sent in to the National Treasurer with chapter dues annually by November 1.

Your chapter president should have each member fill out a profile sheet at the beginning of every term (semester, quarter, or trimester). Forms should be returned to the VP Collegiates by the second week of each term.
